National Resources
The American Bar Association provides a free smart phone app to store and share your important health care wishes, or the $3.99 version allows you to store documents for numerous family members. Click here
Caring Connections is a program of the National Hospice and Palliative Care Organization (NHPCO) and is a national consumer and community engagement initiative to improve care at the end of life. Caring Connections offers a free printable booklet outlining many facets of end of life decisions in a friendly Q&A format.
Caring Connections also offers free printable forms by state, and they have a Helpline to ask questions at 800-658-8898.
Consumer Reports, as part of its ongoing effort to provide consumers with unbiased and useful information for living, includes in their December 2014 issue “Planning for a Beautiful Death”, an article with details on needed documents, caregiver resources, and lots of helpful information for having the conversations with your family and friends about your end of life wishes.
