A person is eligible for hospice when their primary physician and our medical director certify a terminal diagnosis with a prognosis of approximately six months to live. It is important that a patient/client and their support systems embrace palliative care rather than curative care. A successful hospice program will always support all aspects of patient care to include the emotional, the spiritual, and the physiological. The Medicare Hospice benefit covers all services specific to hospice care. Our administrative staff can pre-authorize services with your individual health care plan prior to the initiation of services. Medical professionals, friends, and family members or patients/clients can make a referral to our hospice. We are committed to provide a free evaluation within 24 hours of a referral, once eligibility criteria has been determined.

On the most part, “needy”, “poor” and/or “low-income” are not synonymous with homeless or jobless. Most of the families served by 3ABF are classified as the working poor. These are the loved ones of crime victims, AIDS patients, the severely disabled, the elderly on fixed incomes, stillborn infants, babies and children with birth defects or terminal illnesses, runaways, etc. Over 95% of the families we serve are classified as ethnic minorities and reside in the City of Houston. Tender Loving Care Hospice of Texas, formerly known as EHC Hospice, believes that hospice is a special concept of care designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure-oriented treatments. In hospice care, emphasis is on care of physical symptoms and other medical problems associated with terminal illness, rather than on the cure of the disease. Priority is given to symptom control for patients and supportive care for family members.

People often associate hospice care with dying—and that’s scary. But hospice care is actually about life—alleviating pain and making the patient as comfortable as possible, as quickly as possible. And the sooner you ask, the more options you have.
Patients and families often resist the idea of hospice because they think they’re “giving up.” So they miss out on the opportunity to be more comfortable and live with less pain and more independence. Hospice care is not only the doctors and nurses who manage the pain and symptoms, but also the emotional and spiritual support of social workers and chaplains. The Certified Nurse Aides provide practical, down-to-earth help with personal care.

When you are confronted with an illness or injury there is no place you would rather be than in the comfort of your own home. Not in a hospital or institution, but home with familiar surroundings, people, memories, things that are important to you. It is no surprise that health care providers know that patients often recover most effectively when they are at home. However, recovering at home does not necessarily mean you do not need professional care. The most effective home recovery might require hospital-quality care.
For those suffering from a life-limiting illness, Accolade Hospice offers individualized compassionate care to allow people to live out their lives in comfort, with dignity and on their own terms. The heart of Accolade Hospice is to provide clinical excellence with a team approach at home.
